Buoy Healthcare Group providers have opted out of Medicare and Medicaid. We do not bill Medicare, Medicaid, Medicare Advantage, or Medicaid managed care plans, and services are not reimbursable by those programs. Upon request, we may provide a superbill for eligible services that you may submit to your commercial insurance plan for possible out-of-network reimbursement. Reimbursement is not guaranteed. Payment is due at the time of service. This Fee Schedule may be updated with notice, and updated fees will apply to future services.
Prepaid Follow-Up Visit Packages:
Patients may choose to prepay for a package of follow-up visits at a discounted package rate. Prepaid packages are optional. Patients are not required to purchase a package to receive care. Prepaid package visits may be used only for standard follow-up visits with the clinician listed on the package.
Packages may not be transferred to another patient, family member, clinician, or practice. Package visits may not be used for extended visits, urgent visits, outside laboratory costs, imaging, prescriptions, supplements, administrative fees, legal paperwork, disability forms, insurance forms, record review unrelated to a scheduled visit, or services provided by another clinician, practice, facility, laboratory, pharmacy, or vendor.

Package terms: The 5-Visit Package applies to follow-up visits only. It does not include the initial consultation.
Package visits are intended to be used within 12 months of purchase. After 12 months, unused visits are no longer guaranteed at the discounted package rate. Any unused prepaid value does not expire and may be applied as a credit toward future eligible Buoy Healthcare Group PLLC services, subject to clinician availability, clinical appropriateness, and then-current fees.
